Student Survey: Your Immunization Perspective

Hello all,

We are an interdisciplinary group of graduate students from the UNC School of Pharmacy, UNC School of Public Health, and the UNC Kenan-Flagler Business School, working on an innovative project funded by the Eshelman Institute. The primary goals of our project are to increase patients’ comfort level when receiving vaccines and to reduce the liability of healthcare providers in administering immunizations.

You are invited to participate in this short survey about your experience with vaccines because you are either a healthcare provider or a vaccine receiver. The survey should take 5 minutes or less to complete. Your responses will remain confidential and will be strictly used for the research purpose intended. We would really appreciate your time and feedback.

If you are a healthcare provider, please feel free to fill out both from the patient’s and the provider’s perspectives.
